NSW statutory declaration form
A statutory declaration is a written statement which a person swears, affirms or declares to be true in the presence of an authorised witness – usually a JP, a lawyer or a notary public.
A NSW statutory declaration is made under the Oaths Act 1900. There are two alternative formats specified in the Act, in the Eighth Schedule and in the Ninth Schedule. Either of the two formats may be used.

Please note that from 30 April 2012 there are new requirements for witnessing a person’s NSW statutory declaration. The authorised witness must take additional steps to confirm the identity of the person making the declaration, and certify in writing on the declaration that those steps have been completed.
